Appointment Checklist

Please complete the  Patient Information Form & Medical History Form, and the last page of our Privacy Practice Notice on the FORMS page and present them to our receptionist at the time of your appointment.

Please be sure that you also have the following available:
Your current health insurance card is required and will be photocopied at the time of your 1st visit.
If your insurance requires a co-pay, this is expected at the time of checking in.
Please bring any medical records to your appointment as well as any X-ray films, MRI films, CT scans, or other studies that you have had recently.
If a Referral Authorization is needed for your appointment, it is your responsibility to bring a written authorization with you.
 If your claim will be filed under Worker's Compensation or 3rd Party Liability Insurance, you must provide us the following detail:

*Company's Name & complete Address
*Your claim Number
*Adjustor's Name or Contact Person's Name & Phone Number

 If a patient is a minor (18 yrs. or younger), a parent or guardian must accompany the patient to their appointment, or the patient must have written consent from a parent or guardian in order to be seen by a physician.
 If you have no insurance, or if this a liability claims case, a $50.00 pre-payment is required at the time of your visit.
